Det saknas kontrollerade studier om skillnader mellan könen avseende effekt och biverkningar av paracetamol. Kvinnor kan ha en något högre exponering för paracetamol vid samma dos. Den kliniska effekten av detta är oklar.

Background

The prevalence of several clinical pain conditions is higher in women than in men. Differences in pharmacokinetics, sex hormones, stress response, or socio-cultural aspects may be of importance [1-3]. Therefore, sex and gender differences of pain medications are difficult to interpret [4].
Paracetamol is also known as acetaminophen in English literature.

Pharmacokinetics and dosing
Women have 13-15% lower paracetamol volume of distribution and 12-36% lower paracetamol clearance than men [5-7]. However, these sex differences did not remain after weight correction [7]. Half-life did not differ [5].
Female volunteers given paracetamol in follicular or luteal phase were compared to male volunteers (8 men, 8 women). Compared to men, the mean AUC was increased in women by 39% and 51% and the Cmax increased by 48% and 66% in the follicular and luteal phase, respectively [8].
A higher median paracetamol clearance was found in women at delivery compared to postpartum or non-pregnant women (11.9 vs. 6.4 and 8.4 l/h•m2). However, in non-pregnant women there was no effect on paracetamol cleara......

Försäljning på recept

Fler kvinnor än män hämtade ut läkemedel innehållande paracetamol (ATC-kod N02BE01) på recept i Sverige år 2021, totalt 751 450 kvinnor och 473 197 män. Det motsvarar 146 respektive 91 personer per tusen invånare. Andelen som hämtat ut läkemedel ökade med stigande ålder hos båda könen. I genomsnitt var läkemedel innehållande paracetamol 1,5 gånger vanligare hos kvinnor [10]. Paracetamol kan även köpas receptfritt och då saknas information om användarens kön. Totalt köps 44 % av all paracetamol inom denna ATC-kod på recept [11].
